Which question types can I use?

Every question block in Surbee, what respondents see, and what answer it collects.

Add a question from the block picker: select a block, click Insert, then pick a tile. The common types are shown first, and More question types reveals the rest. The names below match the picker.

Any question can be marked Required, and any question can have a subtitle under it (More options, then Subtitle) for instructions or context.

Text

Text (short answer) gives respondents a one-line box. Use it for names, short facts and single-word answers.

Long answer gives a multi-line box for comments and explanations. You can switch a question between the two with the Long answer switch, and choose a boxed or underlined look.

Choice

Choice (multiple choice) lets respondents pick one option from a list. Click + Add option to add options.

Checkboxes let respondents tick as many options as apply. Turning on Multiple answers on a Choice question converts it to Checkboxes, and turning it off converts it back.

Both can include an Other option with a free-text box, and Checkboxes can set a minimum and maximum number of picks. Ask Surbee's AI to add these, for example "Add an Other option to question 3".

Dropdown shows the options in a compact menu, which suits long lists such as countries or departments.

Yes / No is a two-button question. You can switch its wording to True / False.

Image choice shows each option as a picture with a label. Upload an image for each option. Turn on Multiple answers to let people pick more than one.

Scales and ratings

Rating asks for a number on a short scale, 1 to 5 by default, with labels at each end such as "Poor" and "Excellent". You can add or remove numbers and change the end labels.

Net Promoter Score asks how likely someone is to recommend you, on a 0 to 10 scale.

Slider lets respondents drag a handle to a value in a range, 0 to 100 by default, and shows the value they picked.

Likert lists several statements, and respondents rate each one on the same scale, from "Strongly Disagree" to "Strongly Agree" by default.

Matrix is a grid of rows and columns. Respondents answer each row by picking a column. Use + Add row and + Add column to build it.

On a narrow screen, Likert and Matrix questions stack so each row is answered on its own, which keeps them usable on a phone.

Order and dates

Ranking asks respondents to put a list of items in order of preference. They can drag items, or use the move up and move down buttons.

Date opens a date picker. It can also ask for a time.

Files and signatures

Upload file lets respondents attach a file. The size limit is 10 MB by default, and respondents see a message if their file is too large.

Signature asks respondents to sign, with an optional consent statement above the pad. They can draw their signature or choose Type your signature instead, and it can also ask for their full name. A new Signature question starts as required.

Repeating group

Repeating group is a set of fields that respondents can fill in more than once, such as one entry per activity or per household member. See Repeating groups.

Content blocks

These blocks don't collect answers. They help you lay out a section:

  • Paragraph text for instructions, consent wording or context.
  • Custom code for your own HTML, CSS and JavaScript, shown in a sandboxed frame.

Surbee's AI can also add headings, images, videos, dividers, spacing, columns, tables and buttons when you ask for them in the chat.

Choosing a type

  • To count how many people chose each answer, use a choice question.
  • To compare how strongly people feel, use Rating, Likert or Net Promoter Score.
  • To understand why, add a Long answer question after a rating.
  • To collect a list of similar items, use a Repeating group.
